事故描述：1989年2月24日，聯(lián)合航空811號班機由夏威夷檀香山飛往新西蘭奧克蘭。機上共18名機組人員，337名乘客，死亡9人，均為乘客。飛機爬升至22000 - 23000英尺（6700 - 7000米）時，右側(cè)貨艙門突然爆開，造成11張座椅被彈出機艙，9名乘客因此死亡。由于飛機發(fā)生爆炸性減壓，導致飛機應急氧氣供應系統(tǒng)損壞。機組降低飛機高度來獲取氧氣，同時掉頭180°返回檀香山。

發(fā)動機：普惠JT9D-7A

簡記：機長發(fā)現(xiàn)遇到雷暴天氣，通知乘客系好安全帶，讓后續(xù)發(fā)生的事故傷亡人數(shù)降低。據(jù)幸存者描述，當時凡是沒有固定在飛機上的東西全都從大洞吸飛了，所以一定要系安全帶！影片采訪了一位律師和其中一位遇難者的父母。這場空難發(fā)生前不久才發(fā)生了另一起空難，這個律師就負責那場空難打官司，他為了好好放松一下去度假，不曾想空難律師自己遇到了空難。遇難者父母就很牛了，兒子去世了，這對父母堅持要找出失事的原因，后來才發(fā)現(xiàn)是貨艙門的設(shè)計缺陷問題，如果沒有這對父母，這個問題可能還要很久才能發(fā)現(xiàn)或被承認。父親本身好像是一位工程師。據(jù)說被彈出去的遇難者如果被吸進引擎里就能當場死亡，如果是自由落體的話需要足足在空中掉落4分鐘！這個過程太恐怖了。

事故描述：1999年6月1日，美國航空1420號班機由美國達拉斯飛往美國小石城。機上共6名機組人員，139名乘客，死亡11人（乘客10人，機長1人）。飛機試圖在風暴中降落，但機組人員在降落前忘記將擾流板預位，導致飛機沖出跑道并撞向引進燈，最終停在阿肯色河旁，機身斷裂成3截并起火。

事故描述：1998年9月2日，瑞士航空111號班機由美國紐約飛往瑞士日內(nèi)瓦。機上共14名機組人員，215名乘客，共229人，無一生還。飛機駕駛過程中，正副機長聞到駕駛艙內(nèi)有怪異氣味并看到有白煙冒出，機長準備在就近機場做緊急著陸，但飛機各個系統(tǒng)接連失效，最終飛機在大西洋墜毀且粉碎性解體。飛機失火是因為事故前的一次改造：瑞士航空公司為了吸引更多乘客，在這架飛機頭等艙加裝了一套高級娛樂系統(tǒng)，最終娛樂系統(tǒng)電線過熱引燃了PET隔熱層并導致了火災。

機種：麥道MD-11

發(fā)動機：普惠PW4462

簡記：SW-111號班機是涉及MD-11型客機空難中死亡人數(shù)最高的，后來國際上禁止在飛機上使用PET。此次事故也導致了瑞士航空在2001年破產(chǎn)，并于2002年重組為瑞士國際航空。紀錄片里只采訪到了兩個和這起事故有直接關(guān)系的人，一個是當時的空管，一個是失去女兒的父親，在飛機失事后，他搬到了失事海域附近開了一家餐廳。

4. Flying Blind

事故航班：秘魯航空603號班機

事故原因：維修疏失，靜壓孔被膠帶黏住

事故描述：1996年10月2日，秘魯航空603號班機由邁阿密飛往智利圣地亞哥。機上共9名機組人員，61名乘客，共70人，無一生還。飛機凌晨12:40起飛后5分鐘，機組人員發(fā)現(xiàn)飛機儀器失常，飛機管理電腦發(fā)出各種矛盾的緊急信號，如超速和失速同時警告。機組人員準備緊急下降到可以降落的高度，但由于是在夜晚飛行，機組人員看不到任何可以參考的物體以確定實際海拔高度，且機組人員感受到飛機正在失速，但飛機仍發(fā)出超速警告。各種錯誤的信號導致飛機以極快的速度下降，最終墜毀在太平洋海面上。錯誤的資料和矛盾的緊急訊號是因為地勤工人在進行飛機清潔之后，沒有將黏貼在靜壓孔上的膠帶去掉。

發(fā)動機：普惠PW2037

簡記：正副駕駛太難了，盲飛的同時沒有任何有效信息能夠參考，雖然當時也極力要求利馬機場派出飛機協(xié)助降落，但是在協(xié)助導航的飛機起飛前603號航班就墜毀了。當時將靜壓空貼住的地勤人員也因過失致死罪判刑入獄，秘魯航空也因為這次事故在1999年破產(chǎn)。一片不值錢的膠布害了一飛機的人。

5. Cutting Corners

事故航班：阿拉斯加航空261號班機

事故原因：不依規(guī)程保養(yǎng)導致的機械故障

事故描述：2000年1月31日，阿拉斯加航空261號班機由墨西哥飛往美國西雅圖。機上共5名機組人員，83名乘客，共88人，無一生還。飛機在飛行途中水平安定翼卡住并脫落，導致飛機急速下墜18000英尺，垂直墜入太平洋，高速撞擊海面變成碎片。事故原因是飛機機件維修不足導致的過度磨損，飛機水平安定翼螺母的螺紋被磨平。

機種：麥道MD-83

發(fā)動機：普惠JT8D-219

簡記：航空公司為了賺錢將原本的機件維護時間延長了四倍！！導致機件磨損失效。有一位技師在飛機出事前就舉報了阿拉斯加航空公司違反保養(yǎng)規(guī)定的舉動，導致丟了飯碗，但仍沒能避免事故的發(fā)生。最遺憾的是，事故發(fā)生的兩年前這位技師就命令更換261號班機的伸縮螺桿，然而被下一班主管駁回，就這樣261號班機繼續(xù)載客飛行，但距離下一次檢查還有兩年半。就在下一次檢查前的兩個月，261號班機就因為伸縮螺桿而失事。飛機墜毀時，機長和副機長在緊急時刻選擇了避開人口密集區(qū)，被追授英勇金章。這次事故真的讓人生氣，資本家賺人命錢，為了降低成本，飛機維護記錄作假，記錄了根本沒有做過的維護工作。

6. Flying On Empty

事故航班：越洋航空236號班機

事故原因：燃料外泄，溝通不良

事故描述：2001年8月24日，越洋航空236號班機由加拿大多倫多飛往葡萄牙里斯本。機上共13名機組人員，293名乘客，共306人，受傷18人，死亡0人。飛行4小時左右，燃料開始泄露并引發(fā)駕駛艙警告系統(tǒng)，當時機長和副機長只是認為出現(xiàn)了電腦錯誤，因此開啟了兩側(cè)油箱的轉(zhuǎn)換閥，導致燃料流向泄露燃料的發(fā)動機；燃料泄露的28分鐘之后，飛機右側(cè)的2號引擎燃料用盡并熄火，機長宣告進入緊急狀態(tài)并決定用左側(cè)1號引擎全力飛行。13分鐘后1號引擎也熄滅，使得236號班機成為一架巨型滑翔機，上午06:39，飛行員目測到拉日什空軍基地，機長在執(zhí)行一連串的回轉(zhuǎn)、減速、降低高度后，成功以滑翔方式降落在亞速爾群島拉日什空軍基地33跑道。燃料用盡是因為越洋航空維修人員不妥當?shù)木S修工作，液壓油管和燃料管安裝在一起，使液壓油管和燃料管在飛行時相互接觸摩擦，最后破裂。

發(fā)動機：羅爾斯·羅伊斯Trent 772B-60

簡記：這次滑翔降落打破了民航機滑翔飛行最長距離的世界紀錄，雖然造成意外發(fā)生的原因之一是駕駛員的錯誤操作，但該操作本身是正常的合理操作，只是對故障原因出現(xiàn)了誤判，但最終飛機順利降落僅有少數(shù)人受傷，機體也只有小部分傷害，因此機長在回國時受到了英雄式的歡迎。后來法國民航局和美國聯(lián)邦航空局更改了飛行手冊，要求駕駛員在意圖平衡兩側(cè)油箱油量之前，須先確認是否為漏油所致，然后才可開啟兩側(cè)油箱的轉(zhuǎn)換閥。

第一季

1-1 1999年6月1日，美國航空1420

1）降落后停不下來，沖出跑道——駕駛員在飛機降落時忘了將擾流板啟動

2）在當年的民航管理規(guī)范中，有要求“務(wù)必到站”，在此要求下，本該停飛的暴雨天氣，航班強制起飛，導致災難；且民航管理規(guī)范中僅明確“不能飛入雷暴”，但并沒有明確在什么指數(shù)下不能做什么，應該做什么，缺少明確、精準的動作指引。

1-2 1989年2月24日，美國聯(lián)合航空811航班

1）波音747的艙門固定器設(shè)計有問題

2）航運管理當局同時扮演著兩個角色，既要發(fā)展航空業(yè)，又要監(jiān)督飛行安全，當兩者無法兩全其美同時兼顧時，多會為了經(jīng)濟效益放棄安全，因為經(jīng)濟是一眼望得見的，而安全可以憑僥幸。

1-3 2001年8月24日，加拿大多倫多 越洋航空236航班

1）在油箱漏油嚴重時，機長盲目自信，認為油箱不可能有問題

2）右引擎油管爆裂，因為維修過程中操作不當，在執(zhí)飛前5天已知配件有問題，但航空公司仍強制執(zhí)飛

1-4 1998年9月2日，瑞士航空111航班

1）MD-11機艙起火，由頭等艙的娛樂系統(tǒng)使用過程中系統(tǒng)過熱，引起火災

2）機上材料的耐火標準不夠，在火勢一起便迅速蔓延

1-5 1996年10月2日，秘魯航空603航班（波音757）

1）在起飛前清潔工清理機艙衛(wèi)生時，按要求將靜壓孔用貼紙貼上后再進行打掃，但事后忘記將貼紙撕下

2）應該驗收的督查人員失職，請了普通技師代班

3）且因為貼紙顏色與機艙內(nèi)顏色相近，導致正副駕駛再起飛前的例行檢查時也未能發(fā)現(xiàn)

4）所有原因?qū)е嘛w行過程中正駕駛的靜壓孔被堵，在儀表盤傳送高度數(shù)據(jù)時出現(xiàn)錯誤，引起墜機

5）波音公司應承擔預知產(chǎn)品誤用時可能產(chǎn)生的問題，以及是否有修正空間

6）在此次墜機事故中，清潔工人因過失致死罪名入獄，令一個對后果嚴重性知道最少、最無辜的人，承擔了責任。

1-6 2001年1月31日，阿拉斯加航空261航班

事故發(fā)生時，飛機垂直墜落

1）潤滑不足導致伸縮螺桿故障，事后檢查維修工作程序時，發(fā)現(xiàn)工作記錄作假，在記錄本上記錄的是未做過的工作

2）伸縮螺桿在飛機例行檢查規(guī)范的要求中，檢查的間隔時間很長（多年一檢），本該換掉的伸縮螺桿，在當任主管檢查時被記錄在案，但卻被下一任主管駁回并刪除，為了節(jié)省維修成本

3）設(shè)計缺陷：沒有失效的安全設(shè)計
